Chronic disease management plans: Who receives them and do they make a difference to use of health care services and health outcomes?

Project aim(s):

Dr Rosemary Korda from the Australian Centre for Economic Research on Health at The Australian National University leads this study of Study participants with heart disease, diabetes or asthma to determine if health outcomes or health services are impacted by chronic disease management plans. They will compare patterns of health service use—including visits to doctors, hospital admissions and emergency department attendances, cost and mortality —in those who do and do not have a current GP management plan;
