Research application forms now available

The research application forms for the 45 and Up Study have recently been reviewed and updated.  The same application form is now used for both Stage 1 and Stage 2 applications.

There are two forms, depending on the type of access the research project requires.  Researchers wishing to access baseline questionnaire data only should complete an 'Application to use baseline questionnaire data'.  Researchers wishing to use linked data (i.e. data from Medicare Australia or CHeReL) or researchers wishing to conduct a sub-study (i.e. re-contacting a sub-group(s) of the cohort) should complete an 'Application to use linked and/or sub-study data'.
