Application and amendment forms

Application forms for each type of resource can be found by clicking below:

• Baseline questionnaire data
• Linkage studies
• Sub-studies
• Amendment form - requests to amend previously approved projects, including adding new researchers, requesting additional data, changing research questions or modifying plans for analysis.

Any individual or group can use the data held by the 45 and Up Study, provided that ethical and scientific quality criteria are met.

Any person granted access to the 45 and Up Study data will be required to sign a Deed of Confidentiality.
